As Referrals, Assessment & Admissions Manager, this nationwide role will play a pivotal role in building relationships with Orchard Community Care stakeholders, working with them to identify appropriate residential and respite, as well as establishing the demand needs within the local areas, including HSE, Tusla and Hospital Discharge managers to support with new developments.
The successful candidate will be required to pro-actively source pathways to achieve placements by building and managing relationships with internal and external stakeholders and ensuring service sustainability is maintained.
This hybrid role can be based at our Sligo or Dublin offices with the expectation to travel regularly to visit services or attend meetings with stakeholders. Own car and full driving license are essential.
Summary of Responsibilities
- All key stakeholders to be identified and an action plan written for Orchard Community Care, to include the level and frequency of communication needed.
- Building and maintaining positive, strong, collaborative relationships with Orchard Community Care stakeholders (e.g. HSE, Tusla, Social Workers, Discharge Managers) across Ireland.
- Maintaining regular contact with appropriate individuals in HSE and Tusla community teams in order to generate appropriate referrals and understand their needs.
- Ensuring stakeholders are aware of placement availability and identifying people who Orchard Community Care can provide appropriate support for within their social care portfolio. Processing and coordinating all referrals from start to finish. Working with and supporting the Regional Directors, MDT and Assessment Team to progress these.
- As part of the Orchard Community Care house opening team ensure all houses open on time as per the company schedule, and ensure all vacancies are backfilled in a timely fashion across all houses.
- Meeting key KPIs for Orchard Community Care’s portfolio of specialist residential and respite services across Ireland.
- Supporting operational colleagues with the maintenance of the existing funding arrangements for the people we support, ensuring these are kept up to date following care reviews, and are reflective of current support needs.
- Acting as a key and functioning part of the team within the services within your remit. Maintaining positive working relationships with the Operations Teams, and a strong understanding of current service activity.
- Leading on developing Orchard Community Care’s service provision portfolio nurturing strong partnerships with key stakeholders, presenting new opportunities to the Group and coordinating projects from start to finish.
- Promoting Orchard Community Care’s presence across Ireland, by attending conferences, engagement events and where appropriate, hosting external stakeholders to visit the services.
- Working with marketing to ensure all material is up to date and the strategy is aligned to the live availability.
- Maintaining a stakeholder contact database and keeping this up to date.
- Promoting and ensuring the good reputation of Orchard Care Group and the services you support is maintained. Acting as a positive role model, maintaining professionalism and demonstrating a positive attitude towards staff, service users, their families, carers, visitors and others.
In order to fulfil this role to the required standard, individuals will require meet the following criteria:
- You will hold a social care, healthcare or equivalent primary degree to honours level
- You will have at least three year’s post graduate experience of working in social care, in a busy environment with a proven track record of achieving positive outcomes and results for the benefit of the people we support
- To have a broad-based knowledge of current trends within social care across Ireland
- Working knowledge of the key stakeholders and other funders within social care in Ireland.
- Previous experience of working with care providers as well as supporting with/leading on new care solutions developments (desirable).
- Previous experience of developing strong relationships with key and senior people within the HSE & TUSLA
- Excellent people and relationship management skills.
- Experience in a sales role at a senior level, with a proven track record of success.
- Previous networking experience.
- Business minded, with the ability to understand high level financial reports.
- Able to work effectively as part of a team and independently.
- Proactive, and able to manage a heavy workload, whilst prioritising tasks accordingly.
- Able to use own initiative and is self-motivated
